Personalize a Directory of Providers in The Alliance Network

Create a Personalized Provider Directory

Click Find a Doctor to get started:

Go to our Provider Directory

Then:

  1. Select "Create a Personalized Provider Directory"
  2. Select your network
    • If you're not sure which network you belong to, check your employee health benefit booklet, speak to your HR representative or contact the administrator or payer on your insurance card. Our customer service representatives can also help you identify your network. For assistance, call 800.223.4139.
  3. Enter a starting location from which to orient your search
  4. Select "Create Directory"
  5. Enter an e-mail address to send your results to, and select "Submit"

An e-mail message will be sent when your directory has been created. Once created, you can view your personalized directory onscreen, save it to your computer, or print it. Because the file can be very large, printing your directory may not be the best option.
