The Alliance works with more than 9,500 doctors and health professionals such as chiropractors, podiatrists, and mental health providers at more than 2,500 clinic locations and 59 hospitals. We also provide access to many home health care entities.

• If you would like to encourage a doctor or facility to join The Alliance's provider network, please complete a Provider Nomination Form. The Alliance will evaluate the request, and if appropriate, contact the provider and extend an invitation to join.
