Create a Personalized Provider Directory

Once you've clicked on Find a Doctor:

  1. Select the Find a Doctor  button
  2. Select “Create a Personalized Provider Directory”
  3. Select your network 
  4. Enter a starting location from which to orient your search
  5. Select “Create Directory”
  6. Enter an e-mail address to send your results to, and select "Submit"

An e-mail message will be sent when your directory has been created. Once created, you can view your personalized directory onscreen, save it to your computer, or print it. Because the file can be very large, printing your directory may not be the best option.
