Health care organizations in Dane County are looking for your thoughts about the health and wellbeing of your community. As employers, you have a unique understanding of how the health of the community impacts the health of your employees, impacting both productivity and insurance rates.
A Community Collaborative
Health care organizations form Healthy Dane, a community collaborative which includes Meriter Hospital, St. Mary’s Hospital, Stoughton Hospital, UW Health, Group Health Cooperative of South Central Wisconsin and Public Health Madison Dane County.
Healthy Dane collaborates across systems to jointly address the needs of Dane County residents. As part of this work, the organizations have come together to conduct a Community Health Needs Assessment (CHNA), a feature of the Affordable Care Act designed to assist health organizations in better understanding the needs and assets of their community.

A significant feature of the CHNA is public input.
Healthy Dane invites all residents of Dane County to provide their thoughts by completing an online survey. The results of the survey will help Healthy Dane address needs, set goals and work together along with other partners to improve the health and well-being of local residents.
The survey is open until May 31st and is available online in English and Spanish.
